KNOWN MEDICAL PROBLEMS:
Blood Pressure: From time to time I have had problems with diastolic high blood
pressure, although I am not currently on medication for it.
Asthma: Currently on Albuterol and Advair for this
Tendinitis/Carpal Tunnel: pain in hands, wrists, knees, ankles, feet. Currently on Naprosyn for this.
ANY KNOWN ALLERGIES:
Methergine--have a reaction where my hands turn pink, I become disoriented, and blood pressure rises
Prozac--Causes me to become very anxious and suicidal
Zyprexa--causes me to have facial tics
Abilify--causes very severe nausea
CURRENT MEDICATIONS (PRESCRIPTION & OTC)
